The Diabetes Education Program is aimed at adults diagnosed with diabetes. The program includes a one-on-one session with the nurse and dietitian, a 2 day group education session and one-on-one follow up appointments. Patients will learn extensively about diabetes, its treatment, management and potential complications. In addition, topics will include self monitoring of blood glucose, foot care, stress management, nutrition for diabetes, heart health, label reading, among other topics.

The Diabetes Prevention Program is aimed at adults who are diagnosed with Impaired Fasting Glucose and/or Impaired Glucose Tolerance. The program consists of a ½ day group education session on the basics of diabetes, healthy lifestyle and weight control.

The Insulin and Diabetes Program is aimed at adults who have recently started insulin therapy or who require an update on the use of insulin. The program consists of a ½ day group education session. Topics will include all aspects of the self administration of insulin, how your insulin works, treatment of hypoglycemia, carbohydrate counting, healthy lifestyle, among other topics.

Insulin Initiation is aimed at adults who need to start on insulin. It is offered one-on-one with the nurse and the dietitian. It is offered as needed. A physician referral is required for this program only.

All programs have communication protocol in place so that the physician is aware of the patient’s progress and any concerns that the diabetes team have identified.

Diabetes Info Nights are evening programs focused on a topic of interest, chosen by the clients of the clinic. These are held 3 or 4 times each year.

Grocery Store Tours with the Diabetes Clinic’s Registered Dietitian are held 3 or 4 times a year. Learn more about the foods you buy and how to interpret food labels.
