Public Reporting of compliance rates for the Surgical Safety Checklist at KDH
All hospitals in Ontario with Operating Rooms are required to publicly report their Surgical Safety Checklist (SSCL) compliance. The Surgical Safety Checklist covers the most common tasks and procedures that Operating Room teams carry out, and has been shown to reduce rates of death and complications among patients. It is a one-page list of items that the Surgical Team discusses at three key phases.
- Before the patient is given anesthesia with all team members present.
- Before skin incision.
- While all team members are present before closing the patient.
Surgical Safety Checklist compliance is achieved when all members of the Surgical Team have addressed items in each of the three phases of the checklist. The three phases are known as: Briefing; Time Out; and Debriefing. Patient safety and quality of care is of utmost importance at Kemptville District Hospital and staff are diligent in ensuring a safe experience for all of our patients. The SSCL is a tool that ensures open communication between all members of the multidisciplinary team (Anaesthesiologists, surgeons, nursing staff, etc) whereby open discussion takes place to help the team familiarize themselves with your medical history and any pertinent information that may be needed for your care. The SSCL can be compared to a checklist used by an airline pilot prior to take-off. It is a final check prior to commencing surgery to ensure the entire team is ready and confident about proceeding with your procedure. Some examples of items reviewed in each of the three phases are:
The Briefing Phase:
- Verify with patient name and procedure to be performed
- Allergy review
- Medications review
- Operative site, side and procedure
- Lab tests, x-rays
The "Time Out" Phase:
- Patient position
- Operative site and side and procedure
- Antibiotics check
- Any concerns prior to proceeding
The Debriefing Phase:
- Surgeon reviews procedure performed & important events
- Anesthesiologist reviews important items
- Nurse reviews correct counts & documentation
